Employees who need to file a leave of absence must do so by submitting the appropriate leave packet found below. The packet must be submitted to Kristal McGee in the Benefits Office – [email protected]

Please review all CS Leave Policies prior to submitting your request. All leave requests must be approved by CS Human Resources once processed by the Benefits Office. Please see the screen shot below for the location of the leave packet.
