These records may be emailed to [email protected], or dropped off at our CVA administrative offices. If these records are not provided to Collierville Schools, the application will not be processed.

We are excited to announce our partnership with FinalForms, an online forms and data management service. FinalForms will replace DragonFly Max and will continue to allow our families and coaches to complete and sign necessary athletic forms electronically.  FinalForms saves data from season-to-season and year-to-year.  The platform also pre-populates information wherever possible, saving time.

Users may review data at any time to verify accuracy.  All users will be required to sign forms once per year after the open date.  A reminder that all forms and physical examinations completed on or after April 15 will be good for the duration of the following school year.

The Parent Playbook is attached. If any support is required during the process, users may scroll to the page bottom-right and click “Get Help”. Please contact your coach if additional help is needed.

FinalForms is a requirement for all student-athletes in Collierville Schools prior to participation during the 2023-24 school year.
Parents are to register at: collierville-tn.finalforms.com
