Homebound

 Mission Statement

 

The mission of the Collierville Schools homebound program is to provide temporary educational services for all K-12 students who are unable to attend school due to illness or injury. Homebound instruction is modified to meet a student's individual needs. Homebound services are designed for students whose absence from school will be longer than ten consecutive days, as diagnosed by a licensed medical professional.  

 

 

Application Process

Parents must call Collierville Virtual Academy to request a homebound application. ALL sections of the form must be completed, signed, and returned by the physician to Sheryl Johnson at [email protected]. Upon receipt all required documentation, a meeting may be scheduled to further determine student's eligibility and hours of service. 

Please note, applications signed by physician’s assistants, nurses, nurse practitioners, chiropractors, etc. will NOT be accepted. Only appropriately licensed specialists (i.e. M.D., Pediatric M.D., OB/GYN M.D., Orthopedic Surgeon M.D., Psychiatrist, etc.) can successfully complete and submit the application. Students who are seeking homebound services for emotional or psychological disorders (bipolar disorder, depression, anxiety, phobias, etc.) must have the form completed by a licensed clinical psychologist, neurologist, or psychiatrist. Family physicians not licensed in treating psychological disorders may not be the doctor of record in these cases. 

If a student’s condition requires homebound services for a period to exceed thirty (30) days, the parent is responsible for submitting a recertification form prior to the end of the initial homebound period. The recertification form must be obtained from the Collierville Virtual Academy, and an additional meeting will be scheduled to further determine re-entry into the school setting or an extension of homebound services.
